While moving into position for an ambush in the vicinity of La Chau (3) hamlet, Hieu Duc District a Marine in the squad tripped a M-26 grenade which was placed on the trail as a booby trap by the enemy.

The explosion resulted in wounding two of the Marines. Pvt Jimmy Lesaine later died of multiple fragmentation wounds.
